Who is eligible for these shelf rates?

  • Groups with 2-50 eligible employees
  • Groups in Ohio, Kentucky or Indiana
  • Groups are not eligible if they are currently in force with Sun Life or Dental Care Plus Group

Get appointed

If you are not appointed with Sun Life then you will need to get appointed before your first sale. We cannot process your sold case until you are appointed.

Simply visit Sun Life's 'Partner with us' page and complete the appropriate forms. It can take up to 10 days to get appointed.
