File a claim
Q. File a death claim?
A. Call or send your request to obtain the Claims Package. To expedite your claim, include:
- name of the deceased, and the date of death
- contract number(s)
Each beneficiary must complete the entire Claims Package and an original certified death certificate indicating the cause of death must be submitted.

Q. Find out what signatures are needed to make changes to my contract?

Q. Request a statement that includes contract values?
A. Call us or send your request via mail including the contract numbers(s) and owner name. This report provides contract information including the owner’s name, current account value and other investment information. It will be mailed to the contract owner's address on record.
