|Participating Provider Organization (PPO) plans||
|Prepaid/DHMO Dental Plans||This plan design offers employees and their families access to a range of dental services from in-network providers at fixed copayment amounts with no deductibles or waiting periods. Our prepaid plans are only available in select markets.|

(for companies with 20+ employees)
|Gives employees the choice of two plans – a basic plan, and an enhanced plan with additional coverage or riders.|
|Voluntary plans||All our Dental plan designs can be offered on a voluntary basis (employees pay all or part of the cost), with enrollment support from Sun Life, including plain language materials and personalized enrollment kits.|
|Administrative Services Only||For companies that self-fund their dental plans, Sun Life can adjudicate and administer employee claims (available to groups with 50+ employees).1|
Depending on the plan design, coverage can range from routine dental visits to more complex dental care. Customizable plan features, such as deductibles, coinsurance levels, plan maximums, and benefit waiting periods, help employers control costs. All plans include our basic Lifetime of Smiles® program that helps improve the overall health of your employees. 2. Our network is made available through dentists under access arrangements with Dental Health Alliance, L.L.C.® (DHA®) and other dental PPO networks.
This dental insurance does not provide coverage for pediatric oral health services that satisfies the requirements for “minimum essential coverage” as defined by The Patient Protection and Affordable Care Act (“PPACA”).
