Dental and vision care can help with employees’ smiles, eyesight and overall health, too. We make it easy for them to use their benefits by offering access to two of the largest nationwide networks of dental and eye-care providers1,2. This also helps them reduce their out-of-pocket costs so they have even more flexibility in spending their benefit dollars!

Our full-range of plans provide you with choices

Our flexible plan options allow you to create the plan that works for your group. Combine our dental and vision plans together for an all-around great package. Employees can even access their plan benefits and find a dental or vision provider on the go with our mobile app – Benefit Tools.

Our traditional fully-insured PPO dental plans include access to our large nationwide network of dentists1. Plan options can include Maximum Allowable Charge plans, also known as MAC plans, Active plans and Passive plans. All plans include Lifetime of Smiles®, our dental health program dedicated to improving the smiles of our members for a lifetime! The program includes:

  • Brush biopsies for early detection of oral cancer.
  • Genetic testing to help identify individuals who are at genetic risk for gum disease.5
  • Periochips to control bacteria and reduce the size of periodontal pockets.5,6
  • Online Dental Health Center that offers members information on preventive dental care.

Clients looking for superior coverage can add select additional features, such as Preventive Max Waiver® or Family Share Max®. Learn more.


Prepaid/DHMO plans give members access to a range of dental services
from in-network providers. Plans offer:

  • Fixed copayment schedule so you know costs up front
  • No deductibles, maximums or waiting periods
  • Preventive services, like cleanings, at low or no cost
  • Specialist services, such as orthodontia, are available at discounted rates

Our Vision insurance plans are designed to help keep employees’ eyes healthy so they can stay productive at work and in life. All of our plans include a comprehensive eye exam because providers can detect signs of chronic health conditions, such as diabetes and high blood pressure, before symptoms arise.

Coverage can include:

  • Savings on eye exams and eyewear
  • Discounts on laser vision correction

As an added benefit, no ID cards or claims forms are necessary!

This benefit may be added to our dental insurance plans. The vision discount plan offers discounts on exams and the purchase of prescription eyewear provided by in-network eye care providers. Fees are automatically reduced at the time of service – no claim forms to fill out!

Our vision discount plans are available in select markets.

You can also choose from the following flexible plan options:

  • Dual Choice for companies with more than 20 employees
  • Triple Choice for companies with more than 30 employees
  • High/Low for companies with more than 20 employees
  • Administrative Services Only for companies with more than 50 employees7

No matter how large our network is nationally, if we don’t have an employee’s dentist our size doesn’t help them. We offer our Dental Network Optimization Program8 to recruit the dentists that matter the most.

Access to the largest network of private-practice eye care providers in the U.S. through VSP®2.

Many plans also offer the added convenience of additional retail locations, such as Costco Optical, Eye Care Centers of America Inc.TM, and Visionworks. Together, these retailers offer more than 1,000 optical stores across the country.
